Adapting a Needs Assessment

So the context that I have chosen to work with for this class is a group of 16 teenage students learning English in a government funded ESL Classroom within a school.  Most of these students are at CLB 4/5 for writing.  I adapted a Needs Analysis document that I found here at islcollective.com and added a few open ended questions to take inventory of grammatical needs of the students.   As part of the PBLA requirements with this group, an autobiography assessment will also take place and be entered into each students portfolio and I had planned to have this needs analysis compliment that works to assist with course content, incorporating student interests etc. you can find this Assessment here:
